 In an effort to better serve you, below is a checklist of things to prepare before your first visit.

• Fill out and sign all downloadable forms before your appointment.
• An original Consent form will be given to you sign a the time of your appointment.
• Payment in the form of Visa, Mastercard, Discover, Check or Cash is expected at the time service is rendered.
• Please allow an hour and a half to two hours for your first visit and one hour for subsequent visits.
• Please have all forms completed and signed for your first appointment.  Feel free to contact us if you have any questions or concerns.
• Wear something very comfortable and loose fitting.
• Please do not have a heavy meal two hours prior to your treatment.
• Please refrain from alcohol or drug use prior to treatment.

 As a service to all patients, we do accept insurance.  Please call to have us verify your insurance to see what is covered.

We also can work with Flexible Spending accounts and will gladly supply a Superbill to be submitted.
